Below is an e-mail form for you to complete. An engagement letter will be sent to you for your review and signature if the terms are acceptable to you. The form documents package is $800 (discounted if you choose to retain my services for administration) and if administration is desired that is $80/month for 1-10 optionholders (a fuller rate structure accompanies the engagement letter). Until an engagement letter is executed there is no agreement for services.
